1. Introduction.

Birth Justice Warriors (BJW) is an organization – founded by Dr. Nellie Taylor-Walthrust and Dr. Martine Hackett dedicated to addressing birth disparities and health related issues in Nassau County, New York. In Nassau County, women of color are four times more likely to die during childbirth than their white counterparts (Metrofocus, 2019). Despite having access to medical services and having a decent healthcare system/network, this area struggles with women of color receiving adequate care during pregnancy and post-partum. This organization found to provided education programs for the women and families within this region but failed to propose a program/intervention to address this issue directly.

Women of color within the Nassau County area are more likely to encounter birth disparities and health issues related to pregnancy than Caucasian women.

b. Description of the program or intervention to be evaluated, including a statement of the underlying explicit or implicit theory of change for the program or intervention (i.e., the mechanisms through which the intervention is expected to produce its intended outcomes).

The program intervention being proposed is creating an interdisciplinary team that focuses on supporting mothers within a hospital team. Having a team with multiple disciplines can increase support for the mothers who are carrying or postpartum to ensure appropriate follow-up is being made at bedside.

c. Description of the agency environment: including the receptivity of the administration staff, and other stakeholders.

The agency environment would include a team of health professionals that would develop a plan of care for the mothers. Staff would include a midwife, social worker, possible physician, and any other professional needed to transition the mother into post-care.
