This assignment is designed to extend the learner’s use of concept mapping as a tool for clinical care planning. The nursing process conTnues to provide the foundaTon for organizing informaTon and thought, whereas the mapping becomes the process for intenTonal criTcal thinking and clinical reasoning. C OURSE O UTCOMES ±his assignment enables the student to meet the following course outcomes. 1. Incorporate criTcal thinking skills into clinical nursing pracTce. 2. Demonstrate communicaTon skills necessary for interacTon with other health team members and for providing basic nursing care to individuals and families. 3. Demonstrate the nursing process while providing basic care to individuals and families re²ecTng di³erent stages of the life span in the extended care, acute care, and community-based se´ngs. T OTAL P OINTS P OSSIBLE 50 points R EQUIREMENTS Choose an individual for whom you perform nursing care in the clinical se´ng. Aµer performing a complete assessment of the person, create a concept map. You will use the nursing process to map your ¶ndings, create and prioriTze nursing diagnoses, plan intervenTons, and evaluate outcomes of those intervenTons. Use at least two published nursing references (other than textbooks) to support your plan of care. In addiTon to the concept map and in relaTonship to it, you will submit a one-page re²ecTve paper that addresses communicaTon and safety and infecTon control. Based on your clinical experience with the person for whom you provided care, you will re²ect on speci¶c elements of communicaTon used during your interacTon. Finally, you will discuss issues of safety and infecTon control related to the care of the person. Refer to the grading rubric for the detailed expectaTons regarding content. P REPARING THE A SSIGNMENT ±o prepare the concept map, you may use any soµware of your choosing. Your textbooks and the Internet may o³er access to simple and free concept map creaTon tools.
