Here is a fascinating article that highlighted successful efforts at getting a handle on escalating health care costs. This is one of the best articles I have read that describes how medical home models of care will work. It discusses the efforts of several different physicians who have built health care teams that have focused on the ‘sickest of the sick’ – the highest utilizers of health care – and how they have changed how the system provides care/education/support to them. The results are pretty outstanding.
This also demonstrates the role of research in evaluating whether outcomes were achieved.
Also, watch for this physicians name in the news – Dr. Gawande. He is beginning to make real inroads into changing our care from treatment focused to people focused.
